Bridge II Sports mission is to create opportunities for children and adults who are physically challenged to play team and individual sports by providing equipment, developing sports, teams and coaching, thereby helping them to discover tenacity, confidence, self-esteem, and the joy of finding the player within.

Health & Well-Being

  • Health Choice for Children, Children's Health Insurance Program
    Health Choice for Children provides health insurance to eligible children between ages 6 and 18 who have no health insurance coverage.  Eligibility is based on family size and income.  Families may be required to pay an enrollment fee and co-pays for services also based on income.

  • Community Alternatives Program for Children (CAP/C)
    CAP/C is a Medicaid waiver program for eligible children, birth through age 20,  who meet Nursing Facility or Hospital Level of Care criteria.  Parent/family income requirements are 'waived', meaning not considered, and eligibility is determined by the child's income.  Services are provided to allow a child with significant medical needs to remain at home and in the community.   Case managers help families identify appropriate supports including in-home nursing, respite, allowed home and vehicle modifications for accessibility and care giver training.

  • Community Alternatives Program for Disabled Adults (CAP/DA)
    This Medicaid waiver program provides appropriate supports and services to eligible adults who meeting Nursing Facility Level of Care criteria allowing them to remain at home and in the community.  Services include case management, home modifications for accessibility, personal care aid, specialized medical equipment and supplies and respite services, specialized medical equipment and supplies and respite services.

    GSD4K's desire is to afford children and their families the opportunity to partake in experiences and adventures that may otherwise have been unavailable or unattainable, due to limited financial resources or other factors.

  • Meg's Smile Foundation
    Meg's Smile Foundation is a 501 c3 charitable organization that provides special days out or gifts to children affected by serious illnesses who are treated in North Carolina hospitals. Our mission is to bring smiles to the faces of these children and their families while they are going through what can be a very long and emotional journey.

Bridge II Sports

Located in Durham, NC and draws athletes from around the Triangle area. They have lots of sports options for varying abilities.
